Students who live in Independence, Sharp, Stone, or Jackson counties and who are enrolled in the ninth through the eleventh grade at one of the target schools listed below are invited to submit an application for participation in APPLE UB. Interested students may obtain an application form by downloading and printing one from the Printable Application link (must have Adobe Reader) on this page, by picking up a form in the guidance counselor's office at their school, or by submitting an online request so that we can mail a form.

Applicants should complete the Student Section of the application form carefully and completely. APPLE UB staff members review the applicants' responses thoroughly, and careful preparation can make the difference in whether or not an applicant is accepted into the program.

A parent or guardian must complete the Guardian Section. Please note that we cannot process any application that does not provide information on parents' educational background and family income. We are required by the U.S. Department of Education to verify income with a signed copy of the most recent year's income tax return. To see Sample Tax Return Forms, click on the Income Verification link below. Also, the parent or guardian must sign the form authorizing school officials to release the applicant's records to us.

The School Section must be completed by the school's guidance counselor or another school official such as an administrator or teacher. The counselor can provide the necessary transcripts and test scores.
