Any individual who works in the U.S. must have a Social Security Card with a Social Security Number (SSN). Numbers are issued only to individuals who are eligible to be employed in the U.S. The Social Security Card Application (Form SS-5) and additional information are available online at the Social Security Administration website.
What to Bring to the Social Security Administration Office
To obtain an SSN, present the following documents, along with a complete Form SS-5, in person to a Social Security representative. All documents submitted must match exactly (date of birth, spelling of names, etc.) with the Department of Homeland Security's records.
- Form SS-5
- I-94 record
- Form I-20 with DSO signature
- Letter from DSO confirming your Lyon related F-1 status and employment eligibility
- Written verification from employer (Lyon) including the nature of the employment, number of hours, and the date employment is expected to begin
